    RELEASE CONSENT FORM

    I irrevocably give the Ottawa Safety Council and/or its designee(s) the right and permission to film and/or photograph me or my child and to use such videos and/or photographs in connection with the Ottawa Safety Council’s purposes. The Ottawa Safety Council’s right to use includes the right to publish on the internet, incorporate the subject videos and/or photographs into promotional videos, adapt, exhibit, reproduce, edit, distribute and display the subject videos and/or photographs, including promotion and advertising, in all markets, media or technology now known or hereafter developed.

    I understand that the videos and/or photographs could be circulated worldwide. I acknowledge that any videos and/or photographs of me or my child become the property of the Ottawa Safety Council and that I will not receive any payment from the Ottawa Safety Council or anyone else for the use of any such videos and/or photographs.
    I further consent to the use of my name in connection with the videos and/or photographs if needed by the Ottawa Safety Council or its designees. I acknowledge that the Ottawa Safety Council will never use the full name of my child.

    I understand and acknowledge that the Ottawa Safety Council has no control over the downloading, copying or sharing of any such videos and/or photographs by third parties accessing the Ottawa Safety Council’s website or other social media platforms.

    I understand that I may retract my consent to the use of the subject videos and/or photographs in the future. I further understand that any such retraction must be made in writing to the Ottawa Safety Council. I understand and acknowledge that if I do retract my consent, the Ottawa Safety Council or its designees will remove the subject videos and/or photographs from its social media.

    I understand that there are inherent and unforeseen risks involved in consenting to the use of the subject videos and/or photographs by the Ottawa Safety Council or its designees. I have read this document in its entirety and fully understand its terms. I understand that I am giving up substantial legal rights by signing below. I acknowledge that I am signing this document freely and voluntarily and intend my signature to be a waiver and complete and unconditional release of the Ottawa Safety Council and its designees of any and all liability that may occur as a direct or indirect result of the use of the subject videos and/or photographs.

    By clicking the box below, I acknowledge that I have read, understand and agree with the content contained within this Media Consent and Release. Clicking the box below will be deemed to be an electronic signature, and each of the Ottawa Safety Council and I may rely on such electronic execution as though it were an original handwritten signature.






    Tips for recording video on your smartphone

    CHECK YOUR VIDEO'S ORIENTATION

    Be sure to orient your phone horizontally (landscape mode).

    AVOID DISTRACTING BACK LIGHTING

    Avoid having a window or light source behind you, since it will make you appear silhouetted.

    REDUCE MOVEMENT AND SHAKING

    Hold the phone with two hands for stability or use something stable to set it against; better yet if you have a tripod, use that! This will avoid shaky videos.

    MAKE SURE YOU’RE LOUD AND CLEAR

    Most smartphones come with good microphones, just make sure it isn’t covered (including by your hand or fingers as you hold the phone). As well, avoid recording outside if it is windy or near heavy traffic.

    Road safety is a shared responsibility, regardless of how you use the road.
